Each Executive Officer, each member of the State Ethics Commission, and the Executive Director of the State Ethics Commission must file a financial disclosure statement annually. This financial disclosure statement must be filed on or before January 15 or within 10 days after appointment.

Each candidate for Statewide office, State Senator, or State Representative must file a Financial Disclosure Statement with the Candidate Petition and Consent of Candidate Forms. The Financial Disclosure Forms are available on the Secretary of State, Elections Division website on the same links as the Consent of Candidate Forms.
